エッセイ: 恐怖のインフルエンザの季節がやってきた / Essay: Deadly busy winter season embarrassed me on medical records.

今年の冬は12月29, 20, 31日と3日連続で同じ病院で内科外来でした。

アメリカで学生をやっていますが日本でもアルバイトしていたのです。

以前ほうれい線のことを書いた時、なんだか反響が大きくて驚いたので、今回も少しだけ暴露話を書いてみようと思います。

今年の冬も、例年と同じくインフルエンザと急性胃腸炎のオンパレード。紙カルテだったので腱鞘炎になりそうだった。

 

私はもともとみんなが褒める達筆(と自負している)が、外来をやっていると、途端に字がとんでもなく汚くなってくる。ひどくなると自分で何を書いたのか、わからない。。。

 

内科のレジデントだった15年前、まだ電子カルテの導入もなく、紙カルテで必死に外来をしていた頃、初めて上司に、「お前、字、きたね〜なー」と言われた日のショックを今でも忘れない。

 

なので、あの日を繰り返さないべく、書き方も書く内容も工夫する。特に病名。"急性上気道炎"なんて書いてたら、性とか炎の字は潰れて読めない。これだと医療事務が困るので、短縮して"感冒"、それさえも自信がなくなるとひらがなで"かぜ"と書く。だけど、それはあまりにも怠惰と自分で思う。

 

カルテの中身でさえも、医師は聞いたことの半分くらいしかカルテに書かない(書けない)。英語の方がサラサラと病名をかけることも多いが、それは今はカルテ開示という恐怖の儀式の可能性があるため許されない。恐怖といっても診療内容ではなく、字の汚さを指摘される方が恐怖だ。これは自分でもキチガイかなあと思う笑。

 

電子カルテになって、この問題は解決されたのだろうか?

最近はすでに風邪のフォーマットが出来上がっている病院も多く、コピーアンドペーストを習得すれば、あとは数クリックで処方箋発行、病名付けまでできるところも多いが、相変わらずまだクリック数が多いなと思う。

 

感冒の場合、患者さんが話した内容は半分くらいしかカルテには記載されない。昨日は嫌なことがあったので、とか、誕生会で飲みすぎた、とかは風邪にはあんまり関係ない。ましてや職業まで聞き取る必要は大抵の場合必要ない。というわけであまりに患者間での代わり映えがしないため、あとでカルテをみた時に、一番わからなくなるのは実は電子カルテなのである。

 

実は字の汚さとか、カルテのこのあたりに書いたな、とかは診療後数時間は割と覚えてるもので、あとちょっとヒントがあれば、患者さんの顔も何とか思い出せることも多い。なのでやっぱり紙カルテも捨てがたい。

 

本当にとっととロボットを導入したいものである。

My Japanese writing is, I would say, relatively good.  However, on the outpatient care, it turns out to be as bad as it can not to be even read by myself. 

 

I don't forget the moment when I was very disappointed when I was told by my senior that my writing had been very bad!!! So, I am always very cautious about keeping my tiny pride. 

Japnese doctors need to write down the name of the disease after seeing patients for health insurance. When I saw a patient with cold, I don't write "acute upper reapiratory disease" or "viral infection" because it's too long and it sometimes become different language when writing quickly. But, still I feel me as lazy and guilty when I write "cold."

 

Japanese doctors has never enough time to write down what patients said in the room. I tried to write in English and it is easy for me to wirte the record but I found it dose not fit because there is a condition that requires doctors wirte the records easy to understand for patients. Actually, I am scared of my medical record to be open for patient not because of the treatment but my dirty wiriting. I think it is very strange, 
I recognise it. 


Electric medical record system have not changed duration that need for seeing patients. So, doctors uses template for "cold." With templates, the record has no uniquness so it sometimes become hard to remind of how patients were. It is irony that I can remind of patients' faces, diseases, smells, wears when I saw my bad writing even it is after a few hours later. I really hope to introduce a new robot to record everything happend in the outpatient room.
